Entered weight-factor arithmetic

The record names the controlling dosing-weight definition and factor basis. It does not choose actual, ideal, adjusted or another weight, and it does not reinterpret mg/kg/day as mg/kg/administration or split a daily amount into a schedule.

Entered amount (mg on the selected basis) = documented dosing weight (kg) × documented factor (mg/kg)
